You can now nominate someone for who you think deserves to be awarded. The nomination is open now and will close on February 20, 2025.
AWARD CATEGORY: This Award consists of a certificate and a monetary award CAD $1000.00 (One thousand Canadian dollar). /// ELIGIBILITY: 1. The candidates eligible for the lhfc’s Award are those nominated by qualified persons. 2. Who have received an invitation from the lhfc to submit names for consideration. 3. Someone qualified to nominate but have not received invitations may also submit nominations 4. No one can nominate himself or herself. /// NOMINATION GUIDELINES: The following must be submitted to nominate a candidate for this award. 1. Must be someone who has influenced and contributed to change people's lives through his/her work 2. Nominations can be made by any colleague, friend, and employer, Rotary Club and Charity Organization. 3. Curriculum Vitae of the nominee (max two pages), including current or former official title and address. 4. A summary of professional history of nominee how the candidate meets the selection criteria (max 500 words). /// MUST INCLUDE NOTABLE DETAILS. • You may add a letters of support, testimonials, newspaper articles, editorials, etc. • Submission should not exceed 10 pages in total and must be in a PDF format. • Submissions should be clear and concise how the individual has met the above criteria. • Only complete applications will be accepted as per criteria given above. • Nominator must receive approval from nominee before submitting nomination form. • Nominee should be prepared for their names, photos and application to be made public if awarded. • Nominee must be available for a Zoom meeting with love & hope foundation canada’s Board of Directors • Attach a color photo of nominee 50x70mm.
Please nominate an extraordinary allied healthcare professional from CAMRT, ASRT, ISRRT, WRETF, IARRT who has influenced and contributed in the field.
Advancing the science of medical imaging through working experience or research.
Advancing the continuing medical education through online activities and/or
Advocating for increasing educational awareness in his/her profession.
Must be an active medical imaging/theraphy professional and hold an allied healthcare license with place of employment.
